IMPORTANT ANNOUNCEMENT REGARDING PHYSICAL EXAMINATIONS:
As per New York State, all physicals must now be completed and submitted using the new NYS Medical form.
This form can be found by selecting Health Services under the District tab at the top of our website. Then select Health Forms on the left.
